Adenomyosis Complicated by either Tuberculosis or Endometrial Adenocarcnoma



Abstract
The histogenesis of the adenomyosis uteri has heretofore been the theme of much controversy. And in spite of the relatively frequent studies on adenomyosis that have appeared in the literature, comparatively little attention had been given to its occurrence
in conjunction with carcinoma or tuberculosis of the uterus.
This report consist of two cases of adenomyosis, associated with tuberculosis in one
and endometrial carcinoma in another.
The first patient was a 30 years old female who had complained abdominal discomfort, vaginal bleeding, difficulties in urination and defecation for 4 months duration. She -was a nulli-para and her chest was clear on X-ray examination. At operation the uterus was in normal position but two masses, one in the left wall and another in the anterior wall of the uterine body, were noted. The cut surface of the mass showed fibrous and whirl like tissue. Microscopically the lesion was a typical adenomyosis but scattered granulomatous reaction in the stroma of isolated endometrial tissue in the myometrium.
The second patient was a 55 years old female who had complained of vaginal spotting and lumbago for 9 months. At operation a large mass is found in the right wall of the uterine body. Examination of removed uterus showed several small and large myoma masses in the myometrium. The endometrium was irregular and partly friable. Microscopically the endometrium showed well differentiated adenocarcinoma and multiple islands of carcinomatous glands in the myometrium accompanied by normal endometrial stroma. At some area, the islands of endometrial tissue showed no evidence of malignant change but a typical adenomyosis.
